You're right! "Eu tenho diabetes" is correct for "I have diabetes".
Regarding your other questions:
To say "High blood sugar (level)" you can use: "Alta taxa de açúcar no sangue", or "Alta taxa de glicose no sangue" (This last is literally "High blood glucose level", but is very used in Brazil).
For "My blood sugar is low/ falling": "Minha da taxa de açúcar (glicose) no sangue está baixa/ caindo";
"My blood sugar is high/rising": "Minha da taxa de açúcar (glicose) no sangue está alta/ subindo".
